Combined - $95 NHSRA + $225 State Dues + $100 District Dues

*If student member has changed schools please update MMS information upon renewal

  1. NHSRA Application signed by both parents & member

    • ➔ upload to NHSRA MMS + submit to district secretary

  2. NHSRA Minors Release Form-must be notarized and signed by both parents & member

    • ➔ upload to NHSRA MMS + submit to district secretary

Grade & Conduct Form

Since school has already started, this form must be completed and signed by a school official listed on the form to confirm that the student meets the criteria to enter the rodeo.

Social Media Policy Form

Certified Report Card:

A copy of the 4th quarter report card from your 2025-2026 school year is required for first year and returning members. This includes all charter, correspondence and home school programs and must be recognized and approved with the State of Hawaii Board of Education.

If Applicable:

Home School Verification Form

Form 4140 must be filed with the State of Hawaii. If your school is not recognized by the State of Hawaii, include the site where your proficiency test was taken.

Junior High & High School membership will considered “completed” with:

  1. completed online payment

  2. 5 of 8 docs uploaded to MMS (8 uploaded docs if home schooled)

  3. 8 printed docs (10 printed docs if home schooled) delivered IN-HAND TO YOUR DISTRICT SECRETARY NO LATER THAN 20 DAYS PRIOR TO YOUR FIRST SANCTIONED EVENT/RODEO

  4. District + State Secretary review
